Department of Motor Vehicles (DMV) Information
After cataract surgery, you may no longer need to wear eyeglasses to drive. However, your driver’s license may indicate that corrective lenses are required. In order to have your license corrected, you will need to complete the necessary form and take it to your DMV facility.
If you are licensed in Oregon, please and complete the DMV form below.
Please bring the form to your final post cataract surgery appointment so your eye doctor can sign the required form. If your driver’s license was issued outside of Oregon, please contact your DMV office. Enjoy your great vision!
